To make changes to a survey form (such as adding instructions or pop-up menu answers),

1.Make changes to the Fathom document you used to create the survey (see Create a Survey, Add Multiple-Choice Answers for more detail).

Among the things you can do are: add or delete questions, change the wording of questions, change the title of the survey, and change the answer format for one or more of the questions.

2. In the collection’s Survey panel, click Upload Survey. (If you have closed the document since you last uploaded it, you will need to give your username and password again.)

3. Remember to save your changes.
